This form can be used to register up to 2 adults and up to 5 children from the same family.

These plans help us provide safe and appropriate care during the event. Please email the relevant plan(s) to immaculatamissionschool@gmail.com prior to IMS.

Please also ensure that your asthma reliever medication and/or EpiPen/Anapen is brought to IMS and presented at registration upon arrival. These medications will be sighted at registration and must be kept with you at all times throughout IMS.

Code of Conduct *

Due to the nature of the Springtime Immaculata Mission School, which includes participants being accommodated on site, and under 18s in attendance, we ask all those coming to accept the following Code of Conduct.

During the Springtime Immaculata Mission School 2026 (hereafter “IMS”) I will:

Comply with the rules of the IMS and the directives of the IMS leaders to enable proper functioning of the school and to support the team in their leadership.
Respect the person, property and opinions of each participant, of any other person who I encounter during the IMS (e.g., fellow participants, speakers, those who work at the venue, etc.) and of the venue.
Respect and uphold the teachings of Christ and the Catholic Church.
Refrain from inappropriate sexual conduct, including harassment, verbal abuse etc. This includes refraining from making jokes of a sexual nature.
Not use or possess any illegal drugs or substances and comply with the school being alcohol-free. Smoking, vaping, and the possession, distribution or consumption of alcohol are not permitted on the school.
Report to the leaders any concerns I have about the safety or wellbeing of myself or others.
Take responsibility for myself, my conduct and my personal property.

IMS Waiver

Indemnity *

I understand that while every reasonable precaution will be undertaken to ensure my protection, I hereby release the Immaculata Community and authorised Staff and Volunteers (hereafter “Immaculata Community”) from any and all liability in the event of any injury, accident, misfortune, damage or loss that may occur to me and/or my property while present at the Springtime Immaculata Mission School (hereafter “IMS”). Further, I indemnify the Immaculata Community from and against all loss, including legal expenses, connected with or arising from any claims or demands in relation to my attendance at IMS.

Medical Treatment Consent *

I give permission for the Immaculata Community to obtain emergency medical, hospital or ambulance assistance and/or treatment for myself at any time they consider necessary. I acknowledge that I will be liable for any medical, hospital or ambulance expense incurred in my treatment and I agree to pay those expenses.

Privacy Declaration *

I understand that the Immaculata Community may collect information about me for the purpose of providing promotional material and maintaining ongoing contact with me, and that they will not pass my information on to any other organisation. I consent to my details being used by the Immaculata Community for the promotion of future events and resources via post, phone, email and sms. I can elect to have my details removed from their database at any time. I understand that IMS will be captured in photographs, video and audio, and that the Immaculata Community reserves the right to use this material, including my image and/or voice, for promotional purposes or other resources.

I understand that my disclosure of any accusations will remain confidential, but may prevent my attending the Immaculata Mission School.

Permission to attend the Springtime Immaculata Mission School 2026 & Waiver *

Parental Consent *

I give permission for my child/children to attend the Springtime Immaculata Mission School 2026, to be held at Kildare Catholic College (Coleman St, Wagga Wagga NSW 2650) and to participate in children’s ministry activities. I understand that all reasonable attempts will be taken by the Immaculata Community and authorised Staff and Volunteers to provide adequate supervision of my child/children and that, wherever possible and according to my child’s/children’s age and ability, my child/children also needs to cooperate and abide by the rules of the Mission School and to follow instructions given by leaders.
